We have an exciting opportunity for three Adult Social Workers to join the innovative Transformation Team in Nottingham. Complete Care Act assessments and ensure timely, high-quality outcomes.
Manage and review care packages across Learning Disabilities and Mental Health services.
Use Liquid Logic to maintain accurate, up-to-date case records and documentation.
Attend team meetings and contribute to service improvement strategies.
Collaborate with multi-disciplinary professionals to deliver integrated, person-centred support.
Contribute to innovation and continuous improvement across service delivery.
