Collaborate effectively with multidisciplinary team colleagues, patients, and families for efficient discharge planning. Identify and manage safeguarding concerns following statutory duties. Maintain accurate and timely case records while managing a fast-paced caseload. Essential Requirements: Qualified Social Worker, registered with Social Work England. Experience in hospital discharge, reablement, or adult social care. Strong understanding of the Care Act, Mental Capacity Act, D2A pathways, and safeguarding procedures. Ability to work independently and effectively within a multidisciplinary environment.
