What you will do:
- Provide a professional fieldworker service to service users and their carers, specifically catering to adults dealing with mental health challenges, including those who are terminally ill.
- Conduct comprehensive assessments to determine the needs of individuals with learning disabilities and mental health issues.
- Organise, monitor, and review care packages and other services to ensure they meet the assessed needs effectively.
- Work collaboratively with other professionals to ensure a holistic approach to care and promote independence in line with current legislation and local policy.
- Maintain accurate and timely records to ensure the integrity of information and continuity of care.
Requirements for the Social Worker:
- A degree or equivalent qualification in social work.
- Current registration with Social Work England.
- Significant frontline experience in the UK as a qualified Social Worker within Social Services.
- Strong commitment to safeguarding and promoting the welfare of adults with mental health issues and learning disabilities.
- Ability to work effectively under pressure and manage a complex caseload.
