What you will do:
- Undertake comprehensive Care Act assessments and reviews focusing on adults with mental health and autism-related needs
- Develop, implement, and review person-centred support plans to enhance the quality of life
- Promote independence, choice, and control for individuals receiving support, empowering them to lead fulfilling lives
- Collaborate with health professionals, carers, families, and partner organisations to deliver coordinated services
- Manage a caseload effectively, adapting to changing needs and priorities
- Conduct safeguarding enquiries and risk assessments in necessary situations
- Maintain accurate and timely case records, adhering to professional and council standards
- Participate actively in duty responsibilities and team meetings to contribute to the team's success
- Foster a positive team culture and maintain a visible presence in the office when required
About You:
- You are a seasoned Adults Social Worker driven by the motivation to improve outcomes for adults with autism and mental health needs
- Your expertise in managing complex cases, coupled with strong communication and assessment skills, allows you to work autonomously but collaboratively within a multidisciplinary team
- You possess a valid Social Work England registration, a full driving licence, and can handle travel within the community
