What you will do:

• Undertake comprehensive strengths-based assessments and care management in alignment with statutory duties and local authority procedures.
• Facilitate hospital discharge planning through Discharge to Assess models, ensuring safe and efficient transitions to community settings.
• Collaborate within multidisciplinary teams, including clinicians, therapists, and reablement staff, to coordinate comprehensive care and support plans.
• Conduct safeguarding enquiries and risk assessments following the Adult Support and Protection frameworks.
• Carry out Mental Capacity Assessments and Best Interest decisions, ensuring compliance with all relevant legislation and policies.

Person Specification

Qualifications:

• Degree or Diploma in Social Work (BA, MA, DipSW or equivalent)
• Registration with the Scottish Social Services Council (SSSC) or Social Work England, with transferable candidates considered.

Registration & Compliance:

• In-depth knowledge of key legislation including the Adult Support and Protection, Mental Capacity Act, and the NHS/local authority guidelines.
• Strong understanding of hospital discharge frameworks and integrated care models, essential for holistic social work practice.

Driving is essential for this role; applicants must possess a full UK driving license and have access to a car, as you will frequently travel to various locations to make a genuine difference.
