What you will do:

• Undertake comprehensive strengths-based assessments under the Care Act 2014, including risk assessments and support planning.
• Manage a varied caseload, ensuring timely reviews, interventions, and outcome-focused care plans.
• Lead on safeguarding enquiries in line with Section 42 responsibilities and council procedures.
• Work collaboratively with NHS partners, housing, voluntary sector agencies, and internal teams to coordinate holistic support.
• Promote independence, reablement, and community-based solutions to reduce long-term dependency on services.

Qualifications:

• Degree in Social Work or equivalent recognised qualification.
• Registered with Social Work England.

Registration and Compliance:

• Current Social Work England registration.
• Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and Equality Act 2010.
• Understanding of safeguarding frameworks and Making Safeguarding Personal principles.
