This fantastic opportunity involves a 4-week initial contract at £27.56 per hour, with potential extensions. You’ll be engaging in meaningful work with a full-time schedule from Monday to Friday.

What You Will Do:
- Conduct Care Act assessments and high-quality reviews for adults with autism and learning disabilities.
- Develop person-centred care and support plans, ensuring tailored solutions for individuals.
- Collaborate with families, carers, and partner agencies to deliver comprehensive care.
- Maintain accurate case records and manage workload effectively, ensuring all statutory responsibilities are met.
- Contribute to safeguarding efforts, promoting safety and wellbeing.
- Encourage independence and positive outcomes for service users at every stage.
We Are Looking For:
- Qualified Social Worker status, registered with Social Work England.
- Experience in adult social care, particularly with autism and learning disabilities, is preferred.
- Strong skills in assessment, care planning, and case management.
- In-depth understanding of the Care Act 2014, Mental Capacity Act, and safeguarding.
- Excellent communication skills, enabling effective collaboration.
