What you will do:

• Manage a complex Adult Social Care caseload in an effective and person-centred manner.

• Undertake comprehensive Care Act assessments, reviews, and support planning.

• Complete Mental Capacity Assessments and facilitate Best Interests decision-making processes.

• Lead safeguarding enquiries, assess risks, and implement appropriate protective interventions.

• Prepare detailed reports, assessments, and Court of Protection documentation where required.

• Work collaboratively with health professionals, care providers, and multi-agency partners to coordinate effective support and interventions.

• Monitor, review, and evaluate care packages to ensure desired outcomes are achieved and resources are used effectively.

• Maintain accurate, timely, and legally compliant case records in line with legislation, policies, and professional standards.

• Participate in duty functions and contribute to the ongoing development and improvement of the Adult Social Care service.

• Promote independence, wellbeing, and strengths-based practice in all aspects of your work.
