Rate: 23.75 euros/hour
Role Overview
Hackney Adult Social Care is seeking a dedicated Social Worker to deliver high-quality, person-centred services to adults with care and support needs, their carers, and families. You will complete strengths-based assessments, develop outcome-focused support plans, safeguard vulnerable adults, and promote independence in line with the Care Act 2014 and other relevant legislation.
Key Responsibilities
- Undertake comprehensive Care Act assessments and reviews using a strengths-based approach.
- Develop personalised support plans that promote independence and wellbeing.
- Carry out safeguarding enquiries, risk assessments, and interventions for adults at risk.
- Apply the Mental Capacity Act, DoLS, Best Interest processes, and Making Safeguarding Personal principles.
- Work collaboratively with health professionals, housing, community services, and other multi-agency partners.
- Manage a varied caseload, ensuring timely assessments, reviews, and statutory interventions.
- Provide information, advice, advocacy, and signposting to service users and carers.
- Maintain accurate case records, reports, and documentation using electronic case management systems.
- Prepare court reports and attend legal proceedings where required.
- Participate in supervision, professional development, and continuous learning.
Essential Requirements
- Degree in Social Work (DipSW/CQSW or equivalent).
- Social Work England registration.
- Experience working within Adult Social Care.
- Strong knowledge of the Care Act 2014, Mental Capacity Act, DoLS, and Adult Safeguarding.
- Experience completing strengths-based assessments, support planning, and risk management.
- Excellent communication, report writing, and partnership working skills.
- Ability to manage competing priorities in a fast-paced environment.
Desirable
- Experience with Direct Payments and personalised budgets.
- Best Interest Assessor (BIA) qualification or willingness to train.
- Experience preparing reports for the Court of Protection.
