Hospital Social Worker – Integrated Discharge Team
Location: London Borough of Hackney (Hybrid)
Rate: £ 23.75 per hour
Contract: Temporary / Agency
Working Pattern: 36 hours per week, Monday–Friday
We are seeking an experienced Hospital Social Worker to join the Integrated Discharge Team in the London Borough of Hackney. You will play a key role in facilitating safe, timely hospital discharges by assessing adults with complex health and social care needs, coordinating care packages, and working closely with multidisciplinary teams to promote independence and reduce avoidable hospital admissions.
Key Responsibilities
- Complete comprehensive social care assessments for adults in a hospital setting.
- Develop person-centred care and discharge plans with patients, families, and healthcare professionals.
- Coordinate safe hospital discharges, including arranging care packages, residential placements, equipment, and community support.
- Assess mental capacity, identify safeguarding concerns, and ensure compliance with the Care Act 2014 and Mental Capacity Act 2005.
- Work collaboratively with doctors, nurses, therapists, housing providers, voluntary organisations, and community services.
- Maintain accurate case records, assessments, care plans, and safeguarding documentation.
- Participate in multidisciplinary meetings to support effective discharge planning and patient flow.
Essential Requirements
- Qualified Social Worker (Degree in Social Work, DipSW, CQSW or equivalent).
- Registered with Social Work England.
- Experience in Adult Social Care, preferably within a hospital or integrated discharge setting.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and adult safeguarding legislation.
- Excellent assessment, communication, negotiation, and case management skills.
- Ability to work effectively under pressure in a fast-paced environment.
Working Arrangement
- Hybrid working: 3 days office / 2 days remote.
- Hospital attendance required only when carrying out patient assessments.
