You will complete strengths-based assessments, develop outcome-focused support plans, safeguard vulnerable adults, and promote independence in line with the Care Act 2014 and other relevant legislation.Key ResponsibilitiesUndertake comprehensive Care Act assessments and reviews using a strengths-based approach. Develop personalised support plans that promote independence and wellbeing. Carry out safeguarding enquiries, risk assessments, and interventions for adults at risk. Apply the Mental Capacity Act, DoLS, Best Interest processes, and Making Safeguarding Personal principles. Work collaboratively with health professionals, housing, community services, and other multi-agency partners. Manage a varied caseload, ensuring timely assessments, reviews, and statutory interventions. Provide information, advice, advocacy, and signposting to service users and carers. Maintain accurate case records, reports, and documentation using electronic case management systems. Prepare court reports and attend legal proceedings where required. Participate in supervision, professional development, and continuous learning. Essential RequirementsDegree in Social Work (DipSW/CQSW or equivalent). Social Work England registration. Experience working within Adult Social Care. Strong knowledge of the Care Act 2014, Mental Capacity Act, DoLS, and Adult Safeguarding. Experience completing strengths-based assessments, support planning, and risk management. Excellent communication, report writing, and partnership working skills. Ability to manage competing priorities in a fast-paced environment. DesirableExperience with Direct Payments and personalised budgets. Best Interest Assessor (BIA) qualification or willingness to train. Experience preparing reports for the Court of Protection.
