What you will do:

• Conduct Occupational Therapy assessments in accordance with the Care Act 2014.
• Handle both short-term and long-term cases within Adult Community Services.
• Develop person-centred support plans to meet identified needs.
• Assist adults with physical disabilities, chronic conditions, autism and sensory impairments.
• Implement reablement-focused interventions to promote independence and wellbeing.
• Collaborate with individuals, families and carers to ensure comprehensive care.
• Progress safeguarding enquiries as necessary.
• Facilitate Court of Protection processes and decisions.
• Work collaboratively with health professionals and multidisciplinary teams.
• Perform reviews and reassessments of care and support needs.
• Maintain detailed and accurate records, reports and case documentation.
• Encourage best practice and effective risk management.
