Working as part of an integrated multidisciplinary team, you will be involved in both community and inpatient rehabilitation, contributing to the Intermediate Beds and Urgent Community Response (UCR). You will focus on recovery and reablement therapy, aiming to improve patients' functional outcomes and support their journey back to independence.

What you will do:

• Assess and treat patients within intermediate care and reablement services.
• Complete functional assessments and rehabilitation planning.
• Support admission avoidance and urgent community response pathways.
• Facilitate safe discharge planning and patient flow.
• Assess and recommend equipment to promote independence.
• Deliver recovery-focused occupational therapy interventions.
• Work with patients to maximise functional outcomes.
• Liaise effectively with multidisciplinary teams, carers, and families.
• Complete risk assessments and clinical documentation.
• Support patients within community and intermediate bed settings.

Requirements:

• A degree or diploma in Occupational Therapy and HCPC registration.
• Experience within a Band 6 Occupational Therapy role, including intermediate care and reablement.
• Community rehabilitation and functional assessment experience are essential.
• Strong clinical reasoning and assessment skills.
• Ability to work independently and within multidisciplinary teams.
