Rate: 25.48/ hour

Job Overview

We are seeking an experienced Occupational Therapist to join the Reablement at Home service in Hackney. You will support adults transitioning from hospital to home by delivering therapy-led, person-centred interventions that promote independence and reduce reliance on long-term care.

Key Responsibilities

• Complete functional and occupational therapy assessments.
• Develop and implement reablement therapy plans with person-centred goals.
• Support adults to regain independence in activities of daily living (ADLs).
• Conduct home visits and review progress throughout the reablement period.
• Carry out risk assessments and recommend equipment or adaptations where required.
• Work collaboratively with multidisciplinary teams, including social workers, healthcare professionals, and therapy assistants.
• Maintain accurate clinical records and contribute to safe hospital discharge planning.

Essential Requirements

• Degree in Occupational Therapy.
• Current HCPC registration.
• Experience in Adult Social Care, Community Rehabilitation, or Reablement services.
• Experience with hospital discharge, functional assessments, and home visits.
• Strong knowledge of person-centred and strengths-based practice.
• Excellent communication, assessment, and case management skills.
• Ability to work independently and travel within the community.

Desirable

• Experience working within local authority adult social care services.
• Knowledge of Care Act 2014, safeguarding, and moving & handling principles.
• Previous experience in intermediate care or community rehabilitation services.
