In this challenging but rewarding role, you will work as part of a robust and dynamic social work service. As an allocated social worker, you are responsible for delivering a statutory social work service to those needing specialist mental health services, and you'll support their discharge from hospital. You will be working in partnership with the newly formed mental health hospital discharge hub, tackling statutory duties such as Care Act assessments, s117 aftercare planning meetings, safeguarding, Mental Capacity Assessments, and other social care-related tasks. This fast-paced role requires you to be proactive and able to operate under pressure, ensuring that all facets of the individual’s needs are addressed in the discharge process. Serving as part of a specialized county-wide team, you’ll focus solely on discharge duties, aligned to mental health hospitals catering to people aged 18 and over. The role may require you to manage a range of responsibilities, including conducting strengths-based assessments, planning and reviewing care that addresses eligible needs and outcomes.
What you will do:
- Manage a diverse caseload, working with individuals, families, and carers to assess needs and develop personalised care plans.
- Serve as a professional representative, ensuring safe and effective service delivery for those with complex mental health disorders.
- Collaborate effectively with a range of teams and external agencies, such as the NHS, to coordinate comprehensive support.
- Stay informed on relevant legislation and adhere to professional standards to deliver a high-quality service.
- Develop and implement innovative support plans using local community resources to enhance clients' wellbeing.
