What you will do:

• Provide one-to-one support and keyworking interventions for individuals experiencing substance misuse issues.

• Develop, implement, and review personalised recovery plans focused on individual goals and outcomes.

• Support service users to access treatment, health services, education, training, employment, and community resources.

• Conduct assessments and monitor progress throughout the recovery journey.

• Facilitate group sessions and recovery-focused interventions that encourage peer support and shared experiences.

• Work collaboratively with healthcare professionals, partner agencies, and support services to deliver coordinated care.

• Support harm reduction initiatives and promote safer lifestyle choices.

• Identify safeguarding concerns and follow appropriate procedures to protect vulnerable individuals.

• Maintain accurate, timely, and confidential case records in accordance with organisational policies and regulatory standards.

• Contribute to service development and quality improvement initiatives where required.

• Promote recovery, independence, and empowerment in all aspects of practice.
