What you will do:

Provide one-to-one support and keyworking interventions for individuals experiencing substance misuse issues.

Develop, implement, and review personalised recovery plans focused on individual goals and outcomes.

Support service users to access treatment, health services, education, training, employment, and community resources.

Conduct assessments and monitor progress throughout the recovery journey.

Facilitate group sessions and recovery-focused interventions that encourage peer support and shared experiences.

Work collaboratively with healthcare professionals, partner agencies, and support services to deliver coordinated care.

Support harm reduction initiatives and promote safer lifestyle choices.

Identify safeguarding concerns and follow appropriate procedures to protect vulnerable individuals.

Maintain accurate, timely, and confidential case records in accordance with organisational policies and regulatory standards.
