Are you ready for a new adventure? We have an exciting opportunity in the vibrant community of Ealing for a Band 6 Falls Prevention Therapist specialised in Occupational Therapy. This full-time role boasts a contract duration of three months with the possibility for extension depending on service requirements. With a competitive rate of £27.00 per hour, you will be working 37.5 hours per week within the dynamic Community Falls Prevention Service. Your mission will be to engage with individuals at risk of falls and frailty, offering both individual and group-based interventions.

What you will do:
- Undertake holistic therapy assessments and multifactorial falls risk assessments to support patient safety.
- Complete comprehensive geriatric assessments for frail patients, identifying tailored intervention strategies.
- Deliver innovative and evidence-based falls prevention interventions to improve patient outcomes.
- Independently manage a diverse community caseload, showcasing your organisational skills.
- Facilitate engaging falls prevention exercise and advice groups to empower patient independence.
- Make necessary onward referrals to specialist services, ensuring holistic patient care.
- Monitor outcomes through goal setting and implementing outcome measures.
- Collaborate with multidisciplinary teams to support integrated patient care and minimise falls risk.
Qualifications / Requirements / Person Specification:
- Degree in Physiotherapy or Occupational Therapy.
- Current HCPC Registration.
- Previous NHS experience and community rehabilitation experience.
- Competent in managing frailty and falls caseloads.
- Experience conducting holistic assessments and risk assessments.
- Strong clinical reasoning skills.
- Experience working in multidisciplinary teams and excellent communication skills.
Desirable:
- Experience in a Falls Prevention Service.
- Familiarity with frailty pathways and comprehensive geriatric assessments.
- Experience leading group exercise programmes.
- Experience supervising students or assistants and conducting audits or service development.
