This temporary role, running Monday to Friday 9 am to 5 pm for 37 hours a week, offers a compelling opportunity to utilise your expertise while earning £30 per hour. What you will do:

- Conduct strengths-based social care assessments within D2A assessment bed settings, honing in on the unique needs of each individual.
- Undertake Care Act assessments and reviews, ensuring all processes adhere to regulatory standards.
- Facilitate safe and timely discharge planning, coordinating with various healthcare professionals to streamline transitions and promote independence.
- Evaluate ongoing care and support needs following acute hospital admissions to ensure comprehensive care plans are developed.
- Manage care packages and community-based services, ensuring each plan is tailored to patients' needs.
- Collaborate with nurses, therapists, discharge coordinators, and other MDT professionals to promote seamless care coordination.
- Advocate for independence, focusing on enabling individuals to return to their preferred living environments whenever possible.
- Conduct risk assessments and contribute to safeguarding enquiries to protect and empower service users.
- Work closely with patients, families, carers, and partner agencies, ensuring a holistic approach to care management.
- Maintain precise and timely case records and reports, ensuring all documentation reflects person-centred care goals.
