Band 6 Specialist Physiotherapist within a Community Rapid Response Service in Redbridge, earning £27.00 per hour. This exciting ongoing locum opportunity offers full-time hours of 37.5 per week and the chance to join a dynamic 7-day service covering Redbridge, Havering, Barking and Dagenham. Working within a fast-paced community setting, you will play a crucial role in preventing unnecessary hospital admissions, supporting early discharge, and promoting independence through timely physiotherapy interventions.

What you will do:

  • Provide urgent physiotherapy assessments and interventions to support hospital admission avoidance.

  • Respond promptly to referrals for patients experiencing acute mobility, functional, or rehabilitation needs.

  • Undertake comprehensive assessments of mobility, balance, strength, transfers, and falls risk within community settings.

  • Deliver timely rehabilitation programmes aimed at maximising independence and preventing further deterioration.

  • Develop and implement person-centred treatment plans to ensure patients can remain safe at home.

  • Work collaboratively with multidisciplinary teams to provide coordinated crisis response services.

  • Support early supported discharge programmes and contribute to reducing avoidable hospital readmissions.

  • Recommend and arrange appropriate equipment, environmental adaptations, and exercise programmes.

  • Educate patients, carers, and families on self-management strategies and rehabilitation goals.

  • Maintain accurate clinical documentation and contribute to service evaluation and performance reporting.

  • Participate in service development and quality improvement initiatives where appropriate.

Requirements:

  • Degree or equivalent qualification in Physiotherapy.

  • Current HCPC registration.

  • Previous NHS experience as a Physiotherapist.

  • Community Physiotherapy experience.

  • Experience working within Rapid Response, Urgent Community Response, Intermediate Care, or Admission Avoidance services.

  • Excellent assessment, rehabilitation, and clinical reasoning skills.

  • Ability to work independently and effectively within a multidisciplinary team.

  • Full UK driving licence and access to a vehicle may be required.
