What you will do:

• Deliver specialist neurological assessments and design rehabilitation programmes tailored to individual patient needs.
• Skillfully manage a complex community caseload of neurorehabilitation patients, ensuring optimal patient outcomes.
• Collaborate with a multidisciplinary team to support rehabilitation efforts, prevent admissions, and facilitate timely discharge.
• Perform comprehensive risk assessments and create detailed rehabilitation plans and outcome measures.
• Conduct home visits and interventions in community settings, providing seamless care.
• Mentor and support students, rehabilitation assistants, and junior staff, fostering a learning environment.
• Participate actively in multidisciplinary team (MDT) meetings, contribute to discharge planning, and engage in service development activities.
