What you will do:

• Deliver specialist neurological assessments and design comprehensive rehabilitation programmes.
• Manage a complex community caseload of neuro rehabilitation patients, ensuring high-quality care.
• Collaborate effectively within a multidisciplinary team to enhance patient rehabilitation, prevent unnecessary hospital admissions, and facilitate timely discharge planning.
• Conduct risk assessments, develop detailed rehabilitation plans, and measure outcomes to track patient progress.
• Undertake home visits and community-based interventions, providing essential support to neurological patients in familiar environments.
• Mentor and support students, rehabilitation assistants, and junior staff, contributing to the development of the entire team.
• Actively participate in multidisciplinary team (MDT) meetings, contribute to discharge planning, and engage in service development activities to continually improve patient care.

Qualifications / Requirements / Person Specification:

• Degree in Physiotherapy or Occupational Therapy.
• HCPC registration.
• Previous NHS experience, particularly within neurology and community rehabilitation, is highly desirable.
• Ability to manage complex caseloads independently.
• Strong multidisciplinary team (MDT) working and communication skills.
