What you will do:

- Conduct comprehensive assessments of care and support needs developing implementing and evaluating individualised care plans
- Maintain up-to-date and accurate records ensuring all patient information is documented according to professional standards
- Work collaboratively with multidisciplinary teams including other AHP and nursing teams dietetics teams and external organisations for holistic patient care
- Deliver education and support on disease/condition management promoting self-management and patient independence
- Provide professional leadership to team members guiding and supporting them in advanced nursing care
- Utilise your assessment skills to manage a diverse caseload prioritising patient needs and implementing timely interventions
- Engage in health promotion activities reducing health inequalities and promoting early discharge where possible
