What you will do:

  • Complete comprehensive Pathway Plans to ensure each young adult's journey is personalised and effective.
  • Conduct thorough Child and Family Assessments, updating Case Summaries and completing Chronologies with precision and care.
  • Develop and implement Personal Education Plans (PEP) ensuring that educational aspirations are met and consistently supported.
  • Offer a high standard of guidance and support to young adults facing the transition from care, listening to their needs and advocating on their behalf.
  • Collaborate with a multi-disciplinary team to ensure a holistic approach to care, ensuring services are effectively coordinated.
