What you will do:
- Conduct Care Act assessments, reviews, and support planning to meet the varying needs of adults in the community.
- Undertake safeguarding enquiries to ensure the safety and well-being of individuals.
- Perform Mental Capacity Assessments and participate in Best Interest decision-making.
- Manage Continuing Healthcare (CHC) and Decision Support Tool (DST) work, ensuring all standards are met.
- Handle a complex caseload with a focus on direct work with citizens and carers, fostering strong, supportive relationships.
- Collaborate closely with health professionals and agencies, adopting a multi-agency approach to your work.
- Work in a hybrid capacity, with community visits, home working, and a minimum of one weekly office attendance.

Requirements of the Social Worker:
- Degree or equivalent in social work and current Social Work England registration.
- A significant frontline experience in Social Services within the UK as a qualified Social Worker.
- Possession of a full driving licence, access to a vehicle, and a willingness to travel both inside and outside the council area as required.
