What you will do:

• Undertake targeted reviews of care and support plans in line with the Care Act 2014, ensuring services meet assessed needs and outcomes.
• Assess risk, capacity, and safeguarding concerns, taking appropriate action in line with the Mental Capacity Act and local safeguarding procedures.
• Work collaboratively with individuals, carers, and partner agencies to promote independence and reduce reliance on long-term care services.
• Liaise with health professionals, providers, and internal teams to coordinate effective care planning and service delivery.

Person Specification:

Qualifications

• Degree or postgraduate qualification in Social Work
• Registered with Social Work England

Registration & Compliance

• In-depth knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding adults legislation
• Understanding of Equality Act 2010 and person-centred practice principles
