What you will do:
- Participate in the daily duty rota, managing incoming referrals and responding promptly to service users, carers, and partner agencies.
- Complete Care Act 2014 strengths-based assessments and develop person-centred care and support plans.
- Work collaboratively with health professionals to develop and review Section 117 aftercare plans.
- Attend multidisciplinary team (MDT) meetings, ward rounds, and community reviews to ensure coordinated and holistic care.
- Plan and facilitate safe, timely hospital discharges from acute and psychiatric settings.
- Lead safeguarding enquiries as a Lead Enquiry Officer, ensuring adults are protected from abuse and neglect.
- Undertake Mental Capacity Act assessments and contribute to best interest decision-making where appropriate.
- Complete carers' assessments, identifying support needs and promoting carers' wellbeing.
- Maintain accurate and up-to-date case records in accordance with statutory requirements, council policies, and professional standards.
- Contribute actively to team meetings, service development, and continuous improvement initiatives.
Requirements:
- A recognised Social Work qualification.
- Current registration with Social Work England.
- Previous experience working within Adult Mental Health Services.
- Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act, safeguarding legislation, and Section 117 aftercare.
- Excellent assessment, care planning, communication, and partnership-working skills.
- Experience working within multidisciplinary teams and managing complex adult mental health cases.
