We have an exciting opportunity for a Band 6 Community Psychiatric Nurse to join the Care Coordinator Team in Lewisham. This full-time locum role offers an excellent hourly rate of up to £27.00, depending on experience, with an ongoing 12-week contract. Based at the Lewisham Early Intervention Service on Cordwell Road, you will play a vital role in delivering high-quality community mental health care and coordinating support for service users. What you will do:

• Provide high-quality community psychiatric nursing care as part of a multidisciplinary team, acting as a Care Coordinator for individuals receiving mental health services.
• Undertake comprehensive mental health assessments and develop, implement, and review person-centred care plans.
• Coordinate care across health and social care services, ensuring service users receive timely and effective interventions.
• Maintain accurate clinical records and case management using the EPJS system, ensuring compliance with data protection and professional standards.
• Build positive relationships with service users, families, and carers, providing education, support, and advice throughout their recovery journey.
• Liaise with partner agencies and healthcare professionals to promote integrated working and improve patient outcomes.
• Monitor changes in service users' mental health and escalate concerns appropriately to ensure safe and effective care.

Requirements:

• Current NMC registration is essential.
• Previous experience working as a Community Psychiatric Nurse or within community mental health services.
• Experience in care coordination and multidisciplinary working.
• Knowledge of the EPJS system is desirable.
• A valid driving licence and access to a vehicle are preferred for community visits.
