We have an exciting opportunity for an experienced Integrated Hospital Team Social Worker to join the Take Home and Settle Service in Islington. Working within a multidisciplinary team, you will help individuals regain independence, coordinate appropriate support, and ensure seamless transitions from hospital to home.

What you will do:
- Work collaboratively within a multidisciplinary team to support timely, safe, and person-centred hospital discharges.
- Complete Care Act 2014 assessments, reviews, and strengths-based support planning for adults with a range of complex needs.
- Undertake Mental Capacity Act (MCA) assessments and contribute to Best Interests decision-making where appropriate.
- Manage a complex caseload, ensuring effective care coordination and timely intervention.
- Promote independence by identifying community resources, support services, and preventative interventions that reduce the need for long-term care.
- Lead and contribute to safeguarding enquiries, ensuring vulnerable adults are protected while promoting choice and autonomy.
- Provide professional advice and guidance to colleagues, NHS partners, independent providers, service users, carers, and other stakeholders.
- Maintain accurate, comprehensive, and timely case records in accordance with organisational policies and statutory requirements.
- Participate in multidisciplinary meetings, discharge planning meetings, case conferences, and service development initiatives.
- Engage in regular supervision, reflective practice, and continuing professional development to enhance service delivery and professional practice.
Requirements:
- A recognised Social Work qualification.
- Current registration with Social Work England.
- Previous experience within Adult Social Care, preferably in hospital discharge, integrated hospital teams, or community services.
- Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and relevant adult social care frameworks.
- Excellent assessment, care planning, communication, and partnership working skills.
- Ability to work effectively within a fast-paced multidisciplinary environment.
