Social Worker – Mental Health Discharge Team (DORS) in Horsham, earning up to £38 hourly for a 3-month locum position. What you will do:
- Act as an allocated social worker facilitating hospital discharge for individuals with complex mental health needs.
- Conduct Care Act assessments and s117 aftercare planning meetings, determining needs and ensuring smooth transitions.
- Engage proactively in safeguarding, Mental Capacity Assessments, and Best Interest Meetings.
- Support a small countywide team focused on safe and effective discharge processes.
- Collaborate with family, carers, and healthcare partners, including Sussex Partnership NHS Foundation Trust.
- Work flexibly, often independently, supporting individuals over the age of 18 within either an 18-65 or 65+ age bracket.
- Maintain a caseload, developing and implementing strengths-based support plans to improve patient wellbeing.
- Ensure adherence to Social Work England professional standards and organisational policies in all activities.
- Travel independently across the county, hence a driving licence is advantageous.
