What you will do:

Work within a fast-paced team to provide support to adults in the community dealing with Autism, Acquired Brain Injury (ABI), and physical health conditions.
Collaborate with families and other professionals to develop and implement comprehensive care plans.
Manage and support the duty system and ensure all safeguarding measures are in place.
Utilise your expertise in COPDOL and safeguarding to navigate complex cases efficiently.
Regularly assess client progress and adapt care plans as necessary to meet changing needs.
Communicate effectively and build meaningful relationships with clients to understand their personal needs and goals.
