What you will do:

  • Conduct assessments of need for children and young people with disabilities, including complex care and safeguarding assessments.

  • Develop and implement child-centred care and support plans, ensuring services meet the holistic needs of the child and family.

  • Monitor, review, and adjust care packages, liaising with health, education, and voluntary services to ensure coordinated support.

  • Safeguard children and young people, identifying risks and initiating appropriate interventions under statutory guidance.

Person Specification:

  • Qualifications: Degree in Social Work (BA/MA or equivalent) with evidence of continued professional development relevant to children with disabilities.

  • Registration and Compliance: Registered with Social Work England; knowledge of relevant legislation and frameworks including the Children Act 1989 and 2004, Children and Families Act 2014, and Working Together to Safeguard Children. Enhanced DBS clearance is required.
