What You Will Do:
- Complete assessments and develop tailored care plans for children with disabilities
- Work collaboratively with multidisciplinary teams including health, education, and external agencies
- Advocate for children and families in meetings and decision-making forums
- Monitor and review care plans, adjusting interventions as needs change
- Provide guidance and support to families navigating services and resources
- Maintain accurate and compliant case records in line with statutory requirements
Requirements:
- Qualified Social Worker with Social Work England registration
- Experience working with children with disabilities or within children’s services
- Strong assessment, communication, and advocacy skills
- Ability to manage complex caseloads effectively
