What you will do
- Assessment: Assess the needs of children with disabilities and their families to identify appropriate support and interventions.
- Care Planning: Develop, implement, and review personalised care plans that promote safety, wellbeing, and positive outcomes.
- Family Support: Work closely with children, young people, parents, and carers to provide guidance, support, and advocacy.
- Multi-Agency Working: Collaborate with education, health, and other partner agencies to coordinate services and resources.
- Safeguarding: Identify and respond to safeguarding concerns, ensuring children remain protected and supported.
- Case Management: Manage a varied caseload while maintaining high standards of practice and statutory compliance.
- Report Writing: Prepare detailed assessments, reports, and documentation for reviews, meetings, and court proceedings where required.
- Outcome Focused Practice: Promote independence, inclusion, and improved quality of life for children and their families.
Qualifications / Requirements
- Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
- Registration: Current Social Work England registration.
- Experience: Previous experience within Children's Services, ideally including work with children with disabilities.
- Safeguarding Knowledge: Strong understanding of child protection and safeguarding legislation.
- Assessment Skills: Experience undertaking assessments, care planning, and direct work with children and families.
- Communication Skills: Excellent interpersonal, analytical, and report-writing abilities.
- Partnership Working: Ability to work effectively within multidisciplinary and multi-agency environments.
