What you will do

• Establish and maintain strong relationships with prisoners and relevant agencies to facilitate smooth transitions from custody to the community.
• Conduct assessments to identify prisoner needs and develop tailored support plans.
• Coordinate with external agencies and services to provide comprehensive resettlement support.
• Offer guidance and mentoring to prisoners, helping them access housing, employment, education, and healthcare following release.
• Monitor and report on the progress of individuals after release to ensure continued support and successful outcomes.
