What You Will Do

Build and maintain collaborative relationships with professionals, agencies, and community resources to support individuals.

Conduct comprehensive assessments and plan individual outcomes in partnership with families, carers, and other professionals.

Develop and oversee care plans, delegating tasks and managing caseloads efficiently.

Review and monitor care plans regularly, contributing to service improvements.

Maintain accurate, up-to-date records in line with organisational standards and statutory requirements.

Participate in team meetings and contribute to service development, ensuring equal opportunities are promoted.

Uphold health and safety standards while maintaining confidentiality of service users.

Provide supervision and guidance to students, newly qualified staff, and team members.

Manage complex cases and undertake projects to enhance case management and service delivery.
