What you will do

  • Conduct holistic Occupational Therapy assessments within service users' home environments, evaluating functional abilities and independence needs.
  • Develop and implement person-centred intervention and support plans.
  • Recommend equipment and practical solutions to support independent living.
  • Work closely with service users, carers, and families to identify achievable outcomes.
  • Independently manage a specialist Occupational Therapy caseload and maintain accurate clinical records.
  • Complete reviews and reassessments as needed.
  • Support safe discharge prevention and community independence initiatives.
  • Provide supervision and guidance to Occupational Therapy Assistants.
  • Engage in service development activities to ensure practice is informed by current legislation and evidence-based guidance.

Requirements

  • HCPC Registration: Must be a qualified Occupational Therapist with current HCPC registration.
  • Social Care Experience: Previous experience within Social Care Occupational Therapy is essential.
  • Assessment Skills: Strong assessment and clinical reasoning skills.
  • Caseload Management: Ability to independently manage a specialist caseload.
  • Equipment & Adaptations Knowledge: Experience recommending equipment and adaptations to promote independence.
  • Collaborative Working: Ability to work effectively with service users, carers, families, and multidisciplinary professionals.
  • Desirable Experience: Previous Local Authority experience and experience within a Long Term Team are advantageous.
