Senior Social Worker within the Mental Health Hospital Discharge Team in Ealing, earning £36.00 per hour. What you will do
• Social Care Assessments: Conduct Care Act assessments and reviews for adults with primary mental health needs.
• Support Planning: Develop and implement strengths-based, person-centred care and support plans.
• Hospital Discharge Planning: Facilitate safe and effective discharge planning in partnership with healthcare professionals and community services.
• Mental Capacity Assessments: Undertake Mental Capacity Assessments and Best Interest decision-making in accordance with the Mental Capacity Act 2005.
• Tribunal Reporting: Prepare comprehensive Social Circumstances Tribunal reports to a high professional standard.
• Social Supervision: Act as Social Supervisor for conditionally discharged Section 37/41 patients, ensuring compliance with legal requirements and safeguarding responsibilities.
• Legal Framework Application: Apply relevant legislation including the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 (as amended 2007), and Human Rights Act 1998.
• Court of Protection Work: Contribute to Court of Protection proceedings, including Schedule 21A challenges where required.
• Multi-Agency Working: Work collaboratively with mental health professionals, housing providers, healthcare teams, carers, and partner agencies.
• Record Keeping: Maintain accurate, timely, and legally compliant case records and assessments.
Qualifications / Requirements
• Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
• Registration: Current Social Work England registration.
• Experience: Significant post-qualification experience within Adult Mental Health Services or Mental Health Social Work.
• Hospital Discharge Experience: Experience supporting adults through complex discharge pathways and community reintegration.
• Legislative Knowledge: Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, Human Rights Act 1998, and safeguarding legislation.
• Assessment Skills: Proven ability to complete complex assessments, risk assessments, and support planning.
• Court and Tribunal Experience: Experience preparing reports and contributing to legal and tribunal processes is highly desirable.
• Communication Skills: Excellent written, verbal, analytical, and interpersonal communication abilities.
• Partnership Working: Ability to work effectively within multidisciplinary and multi-agency environments.
