What you will do:
- Conduct outcome-focused assessments for adults with physical disabilities and age-related conditions.
- Perform complex moving and handling assessments within the Front Door Team.
- Evaluate physical dysfunction and functional ability to recommend appropriate complex equipment solutions.
- Provide expert manual handling advice and intervention planning.
- Support safe and effective care planning, managing cases independently.
- Collaborate with multidisciplinary teams and partner agencies to ensure holistic care.
- Maintain meticulous records and assessment documentation to support care delivery.
Requirements:
- Degree or Diploma in Occupational Therapy, with HCPC registration.
- Proven experience as a Senior Occupational Therapist in an Adult Social Care setting.
- Extensive experience with complex moving and handling and equipment assessment.
- Autonomous work capability with strong assessment and clinical reasoning skills.
- Excellent communication and report-writing skills.
Desirable Requirements:
- Previous experience within a Front Door Team or community Occupational Therapy setting.
- Familiarity with Local Authority services and managing complex care assessments.
