What you will do:
- Carry out bed rail reviews ensuring the safety and comfort of service users.
- Complete moving and handling reviews, providing expert guidance on safe practices.
- Undertake moving and handling assessments to identify individual needs.
- Conduct home environment assessments to facilitate safe living spaces.
- Perform transfer assessments, ensuring service users can move safely and comfortably.
- Order and arrange necessary equipment provision for service users.
- Complete triage assessments to prioritise needs and allocate resources efficiently.
- Undertake proportionate care reviews to ensure ongoing suitability of care plans.
- Support safe and independent living within the community through skilled assessments and recommendations.
- Maintain accurate records and assessment documentation for continuity of care.
Requirements:
- Degree or Diploma in Occupational Therapy with HCPC registration.
- Experience in Adult Social Care Occupational Therapy.
- Proficiency in conducting moving and handling assessments and transfer assessments.
- Strong ability to manage an independent community caseload with exceptional communication skills.
Desirable Requirements:
- Community Occupational Therapy experience.
- Equipment and Adaptations experience.
- Reablement experience.
- Local Authority experience.
