What You Will Do:

• Conduct high-quality Care Act assessments, develop personalised support plans, carry out reviews, and implement strengths-based interventions for adults with sensory impairments.

• Lead or contribute to safeguarding enquiries, developing risk management plans and ensuring individuals are included in decision-making.

• Support communication needs by coordinating interpreters, assistive technologies, and accessible information for service users.

• Work collaboratively with healthcare professionals, voluntary sector partners, and other key stakeholders to deliver effective support.

• Manage a complex caseload, using professional judgment to promote independence, empowerment, and wellbeing.

• Contribute to service development initiatives and ensure compliance with relevant legal frameworks, promoting continuous improvement in the Sensory Service.
