Step into an invigorating opportunity as a Qualified Adult Social Worker within our compassionate team in Manchester. With a focus on intermediate care at Willowbank, this 12-month locum position offers the chance to make a tangible difference in the lives of your community members. You will be providing essential support to service users and their families, fostering independence and positive outcomes. This role involves collaborating closely with Manchester Local Care Organisation partners, managing and assessing risks, and developing individualised support plans. Ideal candidates should be available to work two days a week at Willowbank Care Home in South Manchester and one day a week at the Gorton hub in Central Manchester. You should also be available for weekend shifts, scheduled every one in seven weekends. Your role will involve contributing to Multidisciplinary Team discussions and supporting discharge and care planning. Shifts run from 8am to 3:30pm, 8:30am to 4pm on weekdays, and 9am to 4:30pm on weekends.

What You Will Do:

Provide direct support to service users and their families, ensuring compliance with statutory duties and policies for effective outcomes.

Collaborate with MLCO partners and stakeholders to assess and manage risks, design support plans, and deliver care that promotes client independence.

Engage in Multidisciplinary Team discussions, particularly focusing on Estimated Discharge Dates, length of stays, and related matters.

Facilitate discharge and care planning processes by supporting patients and families, managing expectations, and ensuring smooth transitions.

Work closely with therapy staff to compile assessment information, including strategies for high-value care packages.

Keep the flow lead informed on patient progress, escalating social care concerns to senior practitioners and the team manager as necessary.

Participate in the HDTA weekend rota to provide continuous support aligned with unit coverage.
