About the role

An exciting opportunity has arisen for an experienced Band 6 Community Psychiatric Nurse (CPN) to join the STEP Team. This permanent position offers the chance to deliver high-quality, recovery-focused mental health care to adults with complex needs within a community setting.

You will work autonomously while being fully supported by a skilled and multidisciplinary team, contributing to meaningful outcomes for service users through evidence-based practice, care coordination, and therapeutic engagement.

What you will do

  • Manage a caseload of adults with complex mental health needs, providing autonomous and skilled nursing care.

  • Undertake comprehensive mental health and risk assessments, developing person-centred care plans.

  • Implement and review care plans in line with CPA and recovery-focused models, supporting individuals in the community.

  • Deliver evidence-based interventions, including medication management, psychoeducation, and therapeutic support.

  • Monitor mental state and physical health, identifying and escalating concerns appropriately.

  • Act as Care Coordinator, advocating for service users and promoting their independence and recovery.

  • Assess and manage risks related to self-harm, suicide, and safeguarding, in line with policy and best practice.

  • Work within the Mental Health Act and contribute to safeguarding processes.

  • Collaborate closely with the multidisciplinary team, including psychiatrists, psychologists, social workers, and partner agencies.

  • Support transitions from higher-intensity services to community-based care.

  • Provide supervision and guidance to Band 5 nurses, students, and junior staff.

  • Contribute to service development, quality improvement, and clinical governance.

  • Maintain accurate, timely, and professional clinical documentation.
