About the role

We are seeking a dynamic Band 6 Community Psychiatric Nurse (CPN) to join the Primary Care Mental Health Team within the Neighbourhood model in Bromley. This permanent, full-time role offers the opportunity to work at the forefront of integrated mental health care, supporting adults with moderate to complex mental health needs.

You will work closely with GP practices, Primary Care Networks (PCNs), and neighbourhood health and social care teams, focusing on early intervention, relapse prevention, and reducing unnecessary referrals to secondary mental health services. What you will do

Clinical practice

  • Manage a defined caseload of adults presenting with common and severe mental health conditions within primary care.

  • Undertake comprehensive biopsychosocial and mental health assessments, including risk and safeguarding assessments.

  • Formulate, implement, and review person-centred care plans in collaboration with service users and carers.

  • Deliver evidence-based nursing interventions, including brief therapeutic interventions, medication monitoring, relapse prevention, and crisis planning.

  • Provide mental health expertise and consultation to GPs and primary care staff.

Risk management, safeguarding & legal frameworks

  • Identify, assess, and manage risks relating to self-harm, suicide, vulnerability, and safeguarding concerns.

  • Work within the Mental Health Act, Mental Capacity Act, Care Act, and safeguarding frameworks.

  • Escalate concerns appropriately and contribute to multi-agency safeguarding processes.

Integrated neighbourhood & MDT working

  • Work collaboratively within integrated neighbourhood teams, including GPs, social care, voluntary sector partners, pharmacists, and physical health services.

  • Support step-up and step-down pathways between primary and secondary mental health services.

  • Participate in MDT meetings, care coordination discussions, and neighbourhood planning forums.

Leadership, supervision & service development

  • Provide clinical advice, supervision, and mentorship to junior nurses, support workers, and students.

  • Contribute to service improvement, pathway development, and quality assurance initiatives.

  • Support population-health approaches within the neighbourhood model.

Documentation, quality & governance

  • Maintain accurate, timely clinical records using Trust electronic systems.

  • Ensure practice aligns with the NMC Code, Trust policies, and clinical governance standards.

  • Participate in audit, data collection, and outcome monitoring.


Person specification

Essential

  • Registered Mental Health Nurse (RMN) with current NMC registration and evidence of ongoing CPD.

  • Post-registration experience in community, primary care, or secondary mental health services.

  • Strong assessment, risk management, and decision-making skills.

  • Ability to work autonomously within a community-based setting.

  • Sound knowledge of MHA, MCA, safeguarding, and recovery-focused practice.

Desirable

  • Experience working within Primary Care Mental Health, PCNs, or neighbourhood teams.

  • Experience providing consultation to GPs and primary care staff.

  • Experience supervising junior staff or students.

  • Knowledge of South London health and social care systems.
