What you will do:
- Support the delivery of rehabilitation and reablement programmes for adults living in the community.
- Work alongside Reablement Officers, Physiotherapists, Occupational Therapists, and other health and social care professionals.
- Assist with assessments, reviews, and outcome monitoring.
- Support service users to maximise independence and achieve agreed rehabilitation goals.
- Assist with equipment provision and assistive technology interventions.
- Maintain accurate records using electronic care management systems.
- Collaborate with health, social care, and third-sector partners to support ongoing care needs.
Requirements:
- Experience working within a Local Authority
- Experience in Rehabilitation and Reablement
- Strong communication and organisational skills
- Experience supporting adults to achieve rehabilitation goals
- Experience maintaining electronic records
- Understanding of person-centred practice and safeguarding principles
Desirable Requirements:
- Adult Social Care experience
- Community-based rehabilitation experience
- Experience working within multidisciplinary teams
- Knowledge of the Care Act 2014 and Reablement pathways
