What you will do

• Conduct urgent Occupational Therapy assessments to prevent hospital admissions.
• Provide rapid response interventions to support stabilisation during acute decline.
• Assess functional ability and environmental factors within patients' homes.
• Implement strategies and equipment to safely maintain independence.
• Collaborate with multidisciplinary team (MDT) colleagues for crisis intervention.
• Support safe discharge pathways to reduce avoidable delays in care transfer.
• Coordinate with health, social care, and voluntary services for community support.
• Develop person-centred plans to maximise function and mitigate admission risks.
• Monitor outcomes and adjust interventions to support community recovery.
• Maintain detailed clinical records and contribute to performance targets.

Requirements

• Degree in Occupational Therapy.
• HCPC Registration.
• Prior NHS Occupational Therapy experience.
• Post-qualification Occupational Therapy experience.
• Community and Rapid Response experience preferred.
• Skills in falls assessment and risk management.
• Experience in admission avoidance and discharge support pathways.
• Strong clinical reasoning and decision-making abilities.
• Ability to work autonomously in community settings.
• Expertise in multidisciplinary team collaboration.
