What you will do:
- Deliver specialist Occupational Therapy interventions within the Double Handed Care Project.
- Carry out comprehensive OT assessments for adults with complex disabilities and care needs.
- Review existing care packages and identify opportunities to promote greater independence.
- Complete functional assessments and moving and handling assessments.
- Recommend specialist equipment, adaptations, telecare, and assistive technology.
- Produce environmental adaptation specifications and recommendations.
- Train and support care staff to implement care plans effectively.
- Participate in safeguarding enquiries and multidisciplinary care planning.
- Maintain accurate and timely records and reports.
Requirements:
- Occupational Therapy Degree.
- HCPC Registration.
- Exceptional Moving & Handling experience.
- Adult Social Care experience.
- Experience completing complex community Occupational Therapy assessments.
- Experience reviewing care packages and promoting independence.
- Knowledge of the Care Act 2014, Mental Capacity Act, and Housing Grants legislation.
- Experience recommending equipment, adaptations, and assistive technology.
- Strong report writing and clinical reasoning skills.
- Experience supervising Occupational Therapy Assistants or students.
- Experience within review teams or double-handed care projects.
- Experience within Local Authority Adult Social Care services.
