What you will do:

Undertake comprehensive occupational therapy assessments for adults following hospital discharge or during periods of crisis.

Deliver strengths-based interventions that support independence, recovery, and wellbeing.

Develop, implement, and review person-centred rehabilitation plans tailored to individual needs and goals.

Support individuals in regaining essential daily living skills, confidence, and control over their lives.

Assess and recommend appropriate equipment, adaptations, and support services to maximise independence and safety.

Facilitate safe, timely, and effective hospital discharges in collaboration with health and social care professionals.

Work closely with multidisciplinary colleagues to achieve positive and sustainable outcomes for service users.

Undertake risk assessments and develop plans that promote independence while managing identified risks effectively.

Maintain accurate, timely, and professional case records in accordance with HCPC standards and organisational requirements.

Promote strengths-based and outcome-focused practice throughout all aspects of service delivery.
Requirements:

Current HCPC registration as an Occupational Therapist.

Full UK driving licence and access to a vehicle.

Experience within reablement, rehabilitation, community services, discharge planning, or adult social care settings is desirable.

Strong assessment, communication, and problem-solving skills.

Ability to work independently and collaboratively within multidisciplinary teams.
