What you will do:

• Undertake comprehensive occupational therapy assessments for adults following hospital discharge or during periods of crisis.

• Deliver strengths-based interventions that support independence, recovery, and wellbeing.

• Develop, implement, and review person-centred rehabilitation plans tailored to individual needs and goals.

• Support individuals in regaining essential daily living skills, confidence, and control over their lives.

• Assess and recommend appropriate equipment, adaptations, and support services to maximise independence and safety.

• Facilitate safe, timely, and effective hospital discharges in collaboration with health and social care professionals.

• Work closely with multidisciplinary colleagues to achieve positive and sustainable outcomes for service users.

• Undertake risk assessments and develop plans that promote independence while managing identified risks effectively.

• Maintain accurate, timely, and professional case records in accordance with HCPC standards and organisational requirements.

• Promote strengths-based and outcome-focused practice throughout all aspects of service delivery.

Requirements:

• Current HCPC registration as an Occupational Therapist.

• Full UK driving licence and access to a vehicle.

• Experience within reablement, rehabilitation, community services, discharge planning, or adult social care settings is desirable.

• Strong assessment, communication, and problem-solving skills.

• Ability to work independently and collaboratively within multidisciplinary teams.
