What you will do:

Clinical Assessment: Undertake comprehensive mental health assessments, identifying clinical needs, risks, and appropriate interventions.

Triage and Prioritisation: Assess referrals and prioritise individuals based on clinical need, urgency, and risk.

Primary Care Support: Provide rapid access to specialist mental health advice and guidance for GPs and Primary Care teams.

Care Pathway Management: Recommend evidence-based care pathways and support appropriate referrals into specialist mental health services where required.

Service User Engagement: Work collaboratively with individuals and their families, ensuring they receive clear information, guidance, and support.

Education and Training: Deliver formal and informal educational sessions to Primary Care professionals to enhance mental health awareness and knowledge.

Multi-Agency Working: Promote effective communication and collaboration between Primary Care, secondary mental health services, and partner agencies.

Risk Management: Identify, assess, and manage risks, ensuring safe and timely interventions are implemented.

Record Keeping: Maintain accurate, professional, and confidential clinical records in line with organisational and professional standards.

Service Development: Contribute to audits, service reviews, and continuous improvement initiatives to enhance service delivery and patient outcomes.
Requirements:

Registered Mental Health Nurse (RMN) qualification.

Current NMC registration.

Previous experience within mental health services, ideally including assessment, triage, crisis, community, or Primary Care settings.

Strong risk assessment and clinical decision-making skills.

Excellent communication and interpersonal abilities.

Ability to work effectively within multidisciplinary and multi-agency teams.
