What you will do:
- Conduct thorough strengths-based assessments to develop personalised care plans
- Recommend necessary equipment and adaptations to promote independence
- Carry out home visits across Brent, working closely with residents to implement effective reablement strategies
- Collaborate with Social Workers, Care Assessors, Enablers, and health professionals to ensure coordinated, person-centred care
- Maintain strong partnerships with hospitals and community organisations to facilitate seamless service delivery
- Be present in the borough for at least three days each week, working in diverse settings like customers' homes, community locations, partner sites, and the Civic Centre
Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent UK experience as an Occupational Therapist
- Experience within the NHS
- Registered with the HCPC
