What you will do:

  • Work as part of the Housing Solutions team, collaborating with partners across Adult Care Services, Hertfordshire Home Improvement Agency, and local housing entities.
  • Leverage your expertise in the Care Act and Housing Legislation to complete impactful assessments and recommend major housing adaptations.
  • Champion a strength-based assessment and practice approach to enhance independence, health, and well-being, focusing on preventing and delaying the need for Adult Social Care.
  • Employ your creativity and problem-solving skills to support residents in maintaining safety and independence.
  • Utilise your knowledge of local services and resources to facilitate community engagement and support through the Connected Lives Assessment.
  • Conduct functional assessments, providing recommendations for equipment, minor adaptations, and significant housing changes.
  • Apply principles of person-centred care to establish and nurture independence through the Connected Lives Practice.
  • Engage autonomously with cases, enabling independent living and collaborating successfully with various community partners.
  • Exhibit sound self-organisation and maintain a can-do attitude to deliver exemplary support to the community.


Requirements:

  • HCPC registration is required.
  • Being a car driver is essential, as this enables effective community engagement and assessments.
